Short Term Rehabilitation

Sometimes after a person has a surgery, a fall, or maybe just an overall decline in health they need a little boost to get them back to their prior level of functioning. Well this is where Brush Country’s rehabilitation team shines. Here at Brush Country Nursing and Rehabilitation our board certified Physical, Occupational, and Speech Therapists, along with our nursing team work together to create a customized rehabilitation plan specific to the person’s needs while communicating frequently with family along the way to discuss progress and to promote a positive experience. We have countless success stories of where people have benefited so greatly from our therapy services that they were able to move out of our Community back to home feeling stronger and more self-reliant. Our goal with Short Term Rehabilitation is to get you or your loved one back to living life to the fullest. 

We have both Private and Semi-Private Medicare suites available. All Medicare suites have their own bathrooms and TVs equipped with cable at no extra charge.

Brush Country Nursing and Rehabilitation is a licensed Medicare provider. To qualify for Medicare’s Skilled Nursing benefit the following must be met:        

  • Individual was admitted to an acute care hospital for at least 3 midnights
  • Must be admitted within 30 days of the discharge from the hospital or other Skilled Nursing Facility
  • The care and condition must require inpatient services, such as nursing, rehabilitation or both for treatment related to the original hospitalized condition

Long-Term Care

Brush Country Nursing and Rehabilitation offers 3 levels of Nursing Services. The nursing team determines the appropriate care level by assessing the amount of services your loved one needs, during the first five days after admission into our community. This level of care is evaluated quarterly and upon a significant change in condition thereafter. We have chosen to have care levels so we can customize the care being delivered to our residents. The benefit to this is you only pay for what is needed, nothing extra! We do not believe in a blanket approach to ones care, each person we take care of is unique and different so shouldn’t your loved one only pay for what is needed? We think so!      

Brush Country Nursing and Rehabilitation accepts the following payer sources for long-term care: 

  • Medicare 
  • Medicaid
  • Private Pay
  • Most Private / Long Term Care Insurance Policies 

Rates are determined by the level of care and private or semi-private room.
